At Gardens Wholistic Health Center, we value your time and comfort. To make your first visit as smooth as possible, we offer our patient form(s) online so you can complete them in the comfort of your own home or office. This allows Dr. Bill Rice and our team to review your information in advance, helping us focus on your health and treatment goals during your appointment.
All forms are provided in PDF format and can be downloaded directly to your computer. You may fill them out electronically using Adobe Acrobat Reader®, or print them and complete them by hand.
Instructions for Completing Forms
Step 1: Download the Necessary Forms
Click on the file names below to download the appropriate forms for your visit. Each file opens as a PDF document.
If you are interested in any nutritional work, including weight loss, please also complete the Nutritional Counseling packet. This ensures that Dr. Rice has all the information needed to create a complete, personalized care plan.
Step 2: Fill Out the Forms
You have two convenient options:
1. Complete and Submit Electronically
- Open the downloaded file using
Adobe Acrobat Reader®.
- Fill in the required fields directly on your computer.
- When finished, click the “Submit Information” button on the form to email it directly to our office.
2. Complete by Hand
- Download and print the necessary form(s).
- Fill in all required information clearly.
- You may then fax the completed forms to our secure fax line or bring them with you to your appointment.

3. Bring or Send Your Forms
If you prefer not to submit forms electronically, you can bring the printed copies with you on the day of your appointment. Please arrive 10–15 minutes early to allow our team to review your information before your consultation.

Troubleshooting Form Access
If you experience difficulty opening the forms:
Ensure you have the most recent version of Adobe Acrobat Reader® installed on your computer.
If you do not have it, you can easily download it for free from the official Adobe website.
After installation, reopen the form from your downloads folder.
